In June 2023, an Otis Elevator employee in his 20s fell to his death while conducting a repair on his own. The Ministry of the Interior and Safety guidelines stipulate that at least two workers should be present to work on elevator maintenance. Otis Elevator union members reported that they have been demanding for at least two workers to be sent for maintenance jobs but met with repeated refusals from the company. Otis Elevator official said "Our company has maintained elevators in compliance with relevant laws. We have instructed our engineers with a working manual for their safety."

In October 2023, a second death of an Otis Elevator subcontract worker was reported. The subcontracted worker died in September after being hit by a falling rope while working. The union alleged that the supervisor at the construction site where the accident occurred was not trained on the risks of the relevant construction method. This incident is being investigated by the Ministry of Employment and Labor for violation of the Occupational Safety and Health Act or the Serious Accidents Punishment Act.
