Analysis of the causes of accidents and injuries at supervised organizations, 7 March 2025

07.03.2025 — PJSC Chelyabinsk Metallurgical Plant. Supervision of metallurgical and coke-chemical production facilities and sites.

Circumstances:
On 7 March 2025 at 7:38 a.m., at the converter section of the Oxygen Converter Shop of PJSC Chelyabinsk Metallurgical Plant, while cleaning the slag corridor of slag carrier No. 2 of Converter No. 2 (level +0.00 m), a build-up of metallic slag fell from the converter mouth (level +8.00 m). As a result, two workers were injured, and one of them later died from the injuries sustained.

Causes:

• Primary cause: Unsatisfactory organization of work, including failure by managers and specialists of the unit to ensure proper supervision over the work process and compliance with labour discipline.
  
• Contributing cause: Violation of labour rules and workplace discipline by the workers.
  

Measures taken:
A decision was made to hold the responsible officials administratively liable for the violations that led to the accident.
