Rostechnadzor completes investigation into a severe workplace accident at a mining and processing plant, 24 October 2025

The commission of the Upper Don Department of Rostechnadzor has completed its investigation into an accident that occurred at Yakovlevsky Mining and Processing Plant (LLC).

On 18 September 2025, during operations at the hazardous production facility “Underground Mine,” while extracting rock mass in a stoping area, a fragment of backfill material (concrete) fell from the roof on the right-hand side. At the time of the incident, a mining machine operator was nearby and sustained a severe injury after being struck in the back.

Following a review of technical documentation, an inspection of the accident site, interviews with witnesses and officials, and consideration of expert assessments, the commission identified the following causes of the accident:

  • inadequate organization of safe work and worker safety in underground mine workings by the legal entity and officials of Yakovlevsky Mining and Processing Plant;
  • stoping operations conducted with an incomplete face, leaving rock mass in the roof of the working;
  • absence of backfilling of the roof in the stoping area;
  • absence of support or backfilling of the sides of the stoping area;
  • lack of production control by underground mine officials, particularly regarding the support of mine workings;
  • absence of roof-scaling tools in the stoping area.

Administrative proceedings have been initiated against the organization, according to the press service of the Upper Don Department of Rostechnadzor.
